            For County Employees: Have a manager or supervisor submit a Computer Access Request Form to ITS and attach a signed copy of the Acceptable Use Policy.

            For Vendors: Have a Shelby County sponsor submit a Computer Access Request Form to ITS and attach a signed copy of the Acceptable Use Policy, and Confidential Data Vendor Agreement.
